NEW in 2008:   Schools or Clubs registering 24 or more resident players will receive a $20.00/per player cash rebate once all team members check-in (24 or more must check-in to receive rebate).

Deposit:
A $200.00 non-refundable deposit must accompany the registration form to secure a spot in camp. To secure your place in camp, your application and deposit must be postmarked no later than July 1st, 2007

Final Balances: Any balance due after July 1, must be made at the camp on-site registration.  Personal Checks will NOT be accepted after the July 1st date.  Methods of payment at that time are: money orders, cash, or CREDIT CARD (MC or VISA - a $10.00 fee will be added to all Credit card payments).

Health Form:
All campers MUST complete and return the required WMS Health Form. Please download and print the form from this link, or request one by mail, phone, or fax. (This is a 15KB file, which will take about 8 seconds to download over an average household connection of 28.8 Kbs.)

You will need Adobe Acrobat Reader in order to read these files, but since this program comes free with most browsers these days, chances are you will not have to download and install Adobe Acrobat Reader (free!).

To reserve a spot for your team in Team Week, fill out the Team Roster Form, including the team contact and phone number. All individual applications and deposits must accompany the Team Roster form. Incomplete rosters and applications will not guarantee registration until all paperwork is completed. Individual camper balances may be paid individually.
